Project Overview
Since the creation of a single PCT for County Durham, together with one for the Darlington area, and the coming together of 7 District Councils to form a single unitary County Council, there have been a number of significant changes to the health and social care economy in the local area.
In December 2007, CPC and Outcomes UK were commissioned to review the existing models of adult care within each locality across social care, health and housing support and determine the best model of care moving forward to achieve the greatest outcomes to the community in a consistent and value-for-money manner.
Darlington PCT (DPCT) and Durham County Council (DCC) have now asked CPC to review the different models of delivery that had developed over time within the localities, with a view to recommending a single integrated management structure for the management of adult services within the localities.
CPC’s Role
CPC was commissioned to recommend an appropriate management structure for the provision of adult services across health, social care and housing support, both now and in the medium term. In the short term, the focus is on health and social care, with housing support coming on stream in phase 2.
Working with the Integration Project Advisory Board and associated stakeholders, we reviewed the delivery structure of the existing adult integrated care models in County Durham and made recommendations as to the most appropriate common management structure for the future.
We carried out a limited options appraisal and provided an evidence base as part of its recommendations.

CPC provided an evidence-based management structure designed to deliver the health, social care and housing support outcomes for the Durham area.
The resulting recommendation freed up resources to be reinvested in front line delivery of services.
